Why This Matters
Many Central Vermonters. including youth, face challenges that increase risk for substance use, such as social isolation, limited mental health supports, and economic stressors rooted in rural poverty. Our approach:
THRIVE Collaboration
CVPC plays an active role in THRIVE, our region’s Accountable Community for Health (ACH). THRIVE brings together partners from across sectors to address the social determinants of health, the conditions that shape people’s lives and influence their well-being. CVPC ensures that substance use system of care is included in this work, helping THRIVE integrate substance use considerations into initiatives that address housing, food security, mental health, and other root causes of health.
HOPE Initiative
Through the Tufts-based Healthy Outcomes from Positive Experiences (HOPE) framework, CVPC is helping shift the community conversation from adverse experiences to positive ones that build resilience. The HOPE initiative emphasizes the importance of nurturing relationships, safe environments, and community connections as protective factors against substance use. We are growing our local capacity to bring Introduction to HOPE trainings to schools, healthcare teams, and community organizations, spreading evidence-based tools that support health and well-being from the earliest stages of life.
CVPC will continue strengthening prevention through youth engagement, trauma-informed approaches, and efforts that build belonging, connection, and community resilience.
